The statute provides that upon a terminal diagnosis that the physician either implement the directive or make a good faith effort to transfer the patient. The statute requires that physicians keep a copy of any implemented declaration and also provide the health care facility (hospital) with a copy of the declaration. A declaration or revocation is to be made a permanent part of the medical records of the patient.
