            SUMMARY OF STATE LIVING WILL LEGISLATION

TENNESSEE

Synopsis: Tennessee is a strong living will state. The statute
requires that a physician either implement the living will or
inform the next of kin of the patient of his refusal, and then
must assist in transferring the patient. To prepare a Tennessee
living will using the state suggested form, please select living
will preparation from the opening menu. Remember that the
publisher recommends preparing a "uniform" living will in
addition to the form suggested with by the State of Tennessee.
Please also consider executing a living will for any State that
you spend more than a few weeks per year, or where you are a
temporary resident.

A specific form of living will is recommended by the law 

Witness requirements: Number: 2 Relationship: The statute
requires that witnesses not be related to the declarant by blood
or marriage, may not be the attending physician and employees of
the attending physician/hospital where the declarant is a
patient, may not be a person who is entitled to any part of the
declarant's estate and cannot have a claim against the estate.

Recording of the living will is not provided for by the statute 

Revocation of the living will may be made by a separate written
instrument of revocation. A verbal revocation is effective
if made by the patient to the attending physician.
