The following pages provide information pertaining to each family group that includes at least one child. The pages are listed in alphabeti- cal order by the husband's last name because, in most cases, that is the name used by all members of the family group. These records follow a format that has been used by genealogists for many years and serve as a common research document that is in widespread usage. Because the family groups are defined as those having at least one child, and, in some cases, the marriage records may be unavailable, the headings "husband" and "wife" should actually read "father" and "mother". There are also some entries in the forms that do not apply to everyone, such as date of death for individuals that are still living.
